The following are forms needed by groups that have reserved space and/or services at the Warren W. Willis UM Camp. If you have any trouble viewing or printing these documents, please call us at 352-787-4345.

The Guest Use Agreement & Safety Policies must be reviewed by all group leaders before signing the covenant agreement.  It contains all our terms, conditions and policies.

The  Insurance Information Request Form must be completed by all groups that are not part of the Florida United Methodist Conference (Florida UMC churches do not need to complete this form).

The Challenge Course Waiver must be completed by everyone scheduled to participate in either the Low or High Challenge Course.  This form should be completed and turned in at the office upon arrival.

The Waterfront Waiver must be completed by everyone scheduled to participate in canoeing or other waterfront activities.  This form should be completed and turned in at the office upon arrival.
